Fred Form was honored at the Levittown School District Recognition Dinner on April 3. Louise Cassano presented the Levittown Chamber of Commerce Recognition Award to 25-year veteran member, recipient of the Chamber’s Distinguished Service Award and past treasurer, Fred Form.
Since opening his accounting business in Levittown in April of 1987, Fred has been a supporter, volunteer and has served as treasurer at various times on the boards the Levittown Chamber of Commerce, Yours Ours Mine Community Center and the Levittown Community Council. He has volunteered with Levittown and Island Trees School District Advisory boards, and was a member of the Steering Committee for Levittown’s 50thAnniversary.
As a past president and treasurer of Levittown Kiwanis, Fred was awarded the title of Distinguished Kiwanian, Kiwanian of the Year and was the recipient of the prestigious Hixon/Kaiser Award.
Fred hails from Brooklyn, New York and is a graduate of Pace College with a Bachelor of Business Administration in Accounting in 1970. He earned his experience in the field by working for small and mid-sized CPA firms from 1968 to 1987 before coming to Levittown to start his own firm.
He is a Past President of the New York Society of Independent Accounts having also served as Vice President and Treasurer and maintains membership in the National Society of Accountants, the National Association of Tax Professionals.
Fred is married to his #1 employee JoAnn since 1964 with whom he has a son, Andrew, a MacArthur High School graduate and very successful TV and movie producer, who, with his wife movie actress Jordana Brewster, has gifted Fred and JoAnn with two grandsons Julian and Rowan.
