Fred Form was honored at the Levittown School District Recognition Dinner on April 3. Louise Cassano presented the Levittown Chamber of Commerce Recognition Award to 25-year veteran member, recipient of the Chamber’s Distinguished Service Award and past treasurer, Fred Form.


Since opening his accounting business in Levittown in April of 1987, Fred has been a supporter, volunteer and has served as treasurer at various times on the boards the Levittown Chamber of Commerce, Yours Ours Mine Community Center and the Levittown Community Council. He has volunteered with Levittown and Island Trees School District Advisory boards, and was a member of the Steering Committee for Levittown’s 50thAnniversary.

As a past president and treasurer of Levittown Kiwanis, Fred was awarded the title of Distinguished Kiwanian, Kiwanian of the Year and was the recipient of the prestigious Hixon/Kaiser Award. 

Fred hails from Brooklyn, New York and is a graduate of Pace College with a Bachelor of Business Administration in Accounting in 1970. He earned his experience in the field by working for small and mid-sized CPA firms from 1968 to 1987 before coming to Levittown to start his own firm.

He is a Past President of the New York Society of Independent Accounts having also served as Vice President and Treasurer and maintains membership in the National Society of Accountants, the National Association of Tax Professionals. 

Fred is married to his #1 employee JoAnn since 1964 with whom he has a son, Andrew, a MacArthur High School graduate and very successful TV and movie producer, who, with his wife movie actress Jordana Brewster, has gifted Fred and JoAnn with two grandsons Julian and Rowan.

The Levittown Chamber of Commerce held its March general meeting at Domenico’s with guest speaker and fellow chamber member, Aaron Soury who is the managing director at A Neumann Associates, LLC. Aaron presented Business Valuation and The Selling Process.

The Levittown Chamber of Commerce Officers and Board of Directors welcomed new member Steven Giacoppo who owns MJN Technology Services, which is located at 535 S. Broadway in Hicksville. The Levittown Chamber of Commerce was joined by Nassau County Legislator John Ferretti, TOH Town Clerk Sylvia Cabana & Councilman Dennis Dunne Sr, welcoming one of their newest members with a ribbon cutting on January 8. Levittown Oral & Implant Surgery recently opened in Levittown with a state of the art office equipped with the most advanced surgical tools, imaging techniques and monitoring equipment. Services range from wisdom teeth removal, dental extractions, bone grafting and placing dental implants to performing reconstructive jaw surgeries. This allows them to be on the cutting edge in the field of oral and maxillofacial surgery. New Ground Inc. is a unique agency located in Levittown that is committed to educating and empowering families and veterans who are caught in the vicious cycle of homelessness. Their goal is to assist families and veterans in breaking the cycle of homelessness, allowing them to function independently, for generations to come. The Island Trees Public Library’s goal is to be an integral part of the community, meeting the educational, informational, cultural and recreational needs of the community. Chamber Inducts Its Board for 2019 and Honors Community Members

The Levittown Chamber of Commerce, at its annual Installation of Officers and Directors, honored several community members for their service to the Chamber and the community.

Donald Patane was recognized with the President’s Award for the assistance he offered to outgoing President Joan Lynch, who succeeded him as president in 2016. Lynch thanked him for the advice and consistent reliability he provided.

The Levittown Public Library, Levittown Historical Society, and Deejay/Attorney Timothy Aldridge were honored for their Outstanding Community Service. Levittown Library Director Trina Reed accepted on the library’s behalf and, Bob Koenig, vice president of the Historical Society for that organization. Aldridge is well known for his interest in and generosity in providing deejay services throughout the communities of Levittown and Island Trees.

The Chamber recognized Thomas DiIorio as Citizen of the Year. DiIorio is President of the Lions Club of Levittown and active in American Legion Post 1711 and VFW Post 9592. His military service and his service to the veterans’ organizations and the Chamber were cited as the grounds for the recognition given.

Nassau County Legislator Thomas McKevitt, Town of Hempstead Councilman Dennis Dunne, Sr. and Chamber of Commerce Office Manager Tara Cassano were presented with Special Appreciation Awards: McKevitt and Dunne for their commitment and service to the county and town; Cassano for her exceptional quality of service to the chamber.

For her 25 years of service to the Chamber of Commerce, Louise Cassano was recognized with the Distinguished Service Award. Now a director and chair of several committees, Louise served as president of the Chamber in 2004 and 2005.

On a solemn note, the evening’s honors began with presentations to the families of Lois Cornibert and Eric Rosenblum, who passed away during last year. Accepting Lois’ award was her husband Gary, their son Gary and his wife Danielle. Accepting for Eric was his companion Gail Montello.

Attorney and actor Jon Probstein (aka Jon Avner) was installed as the president of the Chamber of Commerce along with Michael Levy, owner of Minuteman Press/Levittown, as 1st vice president; Christine Sweeney, manager of Sterling Bank as 2nd vice president; Robert Ryan, Esq. as Counsel Secretary; and Frank Mormando as treasurer. The named officers include Joan Lynch, as immediate past president.
